It looks like Miller has lost a legand how he directly suffers the attacks of the incident to happen
Can also be seen as Big Boss interrogates Huey or tries to extract some type of information. He even fights against his own comrades, possibly because of the ensuing chaos.
We can appreciate the " horn " of Big Boss provoked by shrapnel from the accident and also, as a curiosity, a certain resemblance to "Hellboy" when anger possesses him and his bloody face is shown. Possibly it's a nod from Kojima to his love for cinema, who knows.
The song playing in the trailer is " Nuclear" by Mike Oldfield, fits perfectly in this installment. He talks about how devastated and desolate everything is and shows the "rage" perfectly in this fantastic trailer.
